When colours are selected without testing
Strolling along the corridors of the museum of Colosseum, I noticed the nice semi-transparent labels of all exhibits. They featured clear red text as headers and white text for further details.
However, when I went a bit further, I noticed something else: these labels were practically unusable when used against a dark wall. You could not see what the exhibit was called - you had to read the white text.

A detail? Maybe. A real life design flaw we can learn from? Definitely.
